Resident Camp Health Form *Mandatory*
The first 3 pages of the health form must be completed by your camper's parent or guardian within 3 months of camp attendance. The health examination section on the last page must be completed by a doctor, physician's assistant or nurse practitioner within 24 months of camp attendance. If you think your camper has a current health exam on file, call the camp registrar at 816-358-8750 to verify. Return your camper's completed health form 30 days prior to her camp session.
Camper Profile *Mandatory*
Help us get to know your camper before she arrives at camp! Fill out this form with your camper and return it 30 days prior to her camp session.
Camper Release Form *Mandatory*
This form identifies and documents transportation arrangements for your camper to travel to and from resident camp. Only authorized adults (other than parents) will be allowed to drop off or pick up your camper from camp. Return your 30 days prior to her camp session.

CIT Application
Fill out this form if you are applying for the Counselor-In-Training program. Both the CIT applications and camp registration must be submitted in order to be considered for CIT.
Financial Assistance for Individuals
Financial assistance is available for girls who would otherwise not be able to participate. Complete this form and submit with camp registration.
Mother/Daughter Overnight Health Form *Mandatory*
This health form must be filled out for each adult or child attending Mother/Daughter Overnight. Bring completed health forms with you to camp. If you have dietary needs, please indicate them on your health form and return it to the Girl Scout Headquarters at least 30 days prior to your camp session. This will ensure your dietary needs will be met.
